脑血管意外后抑郁症量表与CT表现相关研究

应用ＺｕｎｇＤＨＡＭＤＳＣＬ－９０－Ｄ调查了９８例脑血管意外患者的抑郁心理状态，ＣＴ片上观察的指标有ＡＰ％、ＰＰ％、ＬＹ％和病灶部位（皮层或层下）。结果表明：（１）ＰＳＤ的发生率较高，急性期为４３％，慢性期为５５％；（２）不论皮层还是皮层下病灶，急性期左半球前部损伤均产生较高的抑郁程度，ＡＰ％与抑郁程度呈显著负相关，（３）慢性期患者ＣＴ上损伤部位与ＰＳＤ关系不明显，作者的结论是：ＰＳＤ量表得分与有一定关系，ＣＴ可预测ＰＳＤ发生的可能性及抑郁程度大小．

Correlative Study on Rating Scale and CT of Post-stroke Depression

Zung Depression Scale(Zung D),Hamilton Depression Scale(HAMD)and Symptom Check-list-90 Depression SubsCale(SCL-90-D)were used to investigate the psy chologic depression state in 98 patients with cerebrovascular disorder(CVD).The mean dis tance of anterior pole of the lesion to the frontal pole of the brain divided by the longest dis tance from anterior to posterior pole was defined as AP%,the mean distance of posterior poleof the lesion to the occipital pole of the brain divided by the longest distance from anterior toposterior pole of the brain was defined as PP %'The area of each lesion on the largest slice di vided by the area of the brain on slice passing through the lateral ventricles was defined asLV %.On the CT film, AP%,PP%,LV % were measured and the lesion location wasjudged as conical or subcortiCal one.As the results follow:(1) the incidence of Post-strokeDepression(PSD) was 43% in patients with acute CVD and 55% with chronic CVD in chineserespectively.(2)the PSD was more Common and serious in panents with left hemisphereCVD compared with that in patients with right hemisphere CVD,(3)the conical structureshave the same effect on PSD as the subcortical structures,(4) lesion location has less relation ship to PSD in chronic CVD than that in acute CVD.The results indicated that CT can fore cast the PSD incidence and level in acute CVD and may be usefUl to probe into the mechanismof PSD.
