Although mammography screening programs aim to diagnose breast cancer at an early stage, not all tumours are detected during the regular screening examination. This study examines the influence of various characteristics, including geographical residence, on the survival between screen- and interval-detected breast cancers among participants of a public population-based breast screening program in Queensland, Australia. The investigation was performed using the linked population-based datasets from BreastScreen Queensland and the Queensland Cancer Registry for the period of 1997–2008 for women aged 40–89 years at diagnosis. A Bayesian spatial relative survival modelling approach that accommodates rare outcomes in small geographic regions was adopted, with the use of Markov chain Monte Carlo computation, to evaluate the relative excess risk of breast cancer. In the multivariate Bayesian spatial model, higher relative excess risk of mortality was observed in interval-detected cancer (RER = 1.59, 95 % credible interval = [1.33, 1.89]) compared to screen-detected cancer. Higher cancer survival among the study cohort was also observed among younger women (40–59 years), those of non-Indigenous ethnicity, with localised (stage I) tumour stage as well as those not in the work force. There was no independent association with marital status. Moreover, there was no substantive evidence of either measured geographical or latent random spatial inequalities in survival among screening participants across Queensland, meaning the higher survival for screen-detected breast cancer patients compared to interval-detected women was consistent across the state. These results provide suggestive evidence supporting the effectiveness of the BreastScreen Queensland screening program in reaching socio-economically disadvantaged women as well as those living in rural and remote areas of the state, but also highlights the need for any interval cancer awareness programs to be geographically widespread.  相似文献

The purpose of this study was to quantify relationships between season, sediment availability, sediment transport pathways, and beach/foredune morphology at Greenwich Dunes, PEI. This was done for periods ranging from a few days to multiple decades using erosion pins, bedframe measurements, annual surveys, and digital photogrammetry using historical aerial photographs. The relative significance of seasonal/annual processes versus response of the foredune system to broader geomorphic controls (e.g. relative sea level rise, storms, etc.) was also assessed. The data show that there are clear seasonal differences in the patterns of sand supply from the beach to the foredune at Greenwich and that there are differences in sediment supply to the foredune between the east and west reaches of the study area, resulting in ongoing differences in foredune morphology. They also demonstrate that models that incorporate wind climate alone, or even models that include other factors like beach moisture, would not be able to predict the amount of sediment movement from the beach to the foredune in this environment unless there were some way to parameterize system morphology, especially the presence or absence of a dune ramp. Finally, the data suggest that the foredune can migrate landward while maintaining its form via transfers of sediment from the stoss slope, over the crest, and onto the lee slope. Although the rate of foredune development or recovery after disturbance changes over time due to morphological feedback, the overall decadal evolution of the foredune system at Greenwich is consistent with, and supports, the Davidson‐Arnott (2005) conceptual model of dune transgression under rising sea level. Threlkeld Knotts (c. 500 m above sea level) in the English Lake District has hitherto been considered to be a glacially‐modified intrusion of microgranite. However, its surface features are incompatible with glacial modification; neither can these nor the subsurface structures revealed by ground‐penetrating radar (GPR) be explained by post‐glacial subaerial processes acting on a glacially‐modified microgranite intrusion. Here we re‐interpret Threlkeld Knotts as a very large post‐glacial landslide involving the microgranite, with an estimated volume of about 4 × 107 m3. This interpretation is tested against published and recent information on the geology of the site, the glacial geomorphic history of the area and newly‐acquired GPR data. More than 60 large post‐Last Glacial Maximum (LGM) rock–slope failures have significantly modified the glaciated landscape of the Lake District; this is one of the largest. Recognition of this major landslide deposit in such a well‐studied environment highlights the need to continuously re‐examine landscapes in the light of increasing knowledge of geomorphic processes and with available technology in currently active or de‐glaciating environments. Manganiferous rocks in the Mankwadzi area in the southernmost portion of the Kibi‐Winneba metavolcanic belt, one of several Mn occurrences in the Paleoproterozoic Birimian of Ghana, are hosted in hornblende schist and amphibolite. These rocks are, in places, intruded by hornblende dyke. In outcrop, the manganiferous rocks appear to be conformable with the host schist and amphibolite, are macroscopically dark, fine‐grained and structurally massive to distinctly banded. Observed alternating light and dark occasionally macro‐folded bands suggest post‐depositional deformation of both light and dark bands. Microscopic observations revealed that the light bands are dominantly Si‐rich and the dark bands mainly of opaque minerals. Whole rock analyses of the manganiferous rocks show high contents of MnO (16.75–27.4 wt%) suggesting that the opaque minerals are likely rich in Mn. The analyzed rock samples show moderate to strong enrichments in light rare earth elements compared to heavy rare earth elements. Whereas the manganiferous rocks show perceptibly negative Eu anomaly, host hornblende schist and hornblende dyke do not. Eu anomaly in amphibolite samples is, however, uncertain as the three samples analyzed gave positive, negative and no Eu anomalies. Based on the field characteristics, microscopic and geochemical features, we suggest that the Mn occurrence in the Mankwadzi area originated via sedimentary deposition and was later modified by metamorphism, hydrothermal and/or supergene processes similar to manganiferous occurrences at Nsuta and Tambao in the Birimian of West Africa.  相似文献

Liverpool Bay is a region of freshwater influence which receives significant freshwater loading from a number of major English and Welsh rivers. Strong tidal current flow interacts with a persistent freshwater-induced horizontal density gradient to produce strain-induced periodic stratification (SIPS). Recent work (Palmer in Ocean Dyn 60:219–226, 2010; Verspecht et al. in Geophys Res Lett 37:L18602, 2010) has identified significant modification to tidal ellipses in Liverpool Bay during stratification due to an associated reduction in pycnocline eddy viscosity. Palmer (Ocean Dyn 60:219–226, 2010) identified that this modification results in asymmetry in flow in the upper and lower layers capable of permanently transporting freshwater away from the Welsh coastline via a SIPS pumping mechanism. Observational data from a new set of observations from the Irish Sea Observatory site B confirm these results; the measured residual flow is 4.0 cm s−1 to the north in the surface mixed layer and 2.4 cm s−1 to the south in the bottom mixed layer. A realistically forced 3D hydrodynamic ocean model POLCOMS succeeds in reproducing many of the characteristics of flow and vertical density structure at site B and is used to estimate the transport of water through a transect WT that runs parallel with the Welsh coast. Model results show that SIPS is the dominant steady state, occurring for 78.2% of the time whilst enduring stratification exists only 21.0% of the year and enduring mixed periods, <1%. SIPS produces a persistent offshore flow of freshened surface water throughout the year. The estimated net flux of water in the surface mixed layer is 327 km3 year 1, of which 281 km3 year−1 is attributable to SIPS periods. Whilst the freshwater component of this flux is small, the net flux of freshwater through WT during SIPS is significant, the model estimates 1.69 km3 year−1 of freshwater to be transported away from the coast attributable to SIPS periods equivalent to 23% of annual average river flow from the four catchment areas feeding Liverpool Bay. The results show SIPS pumping to be an important process in determining the fate of freshwater and associated loads entering Liverpool Bay.  相似文献

The global fisheries sector has undergone both rapid industrialization and considerable resource depletion. Unlike fisheries in the Northern Hemisphere, the Indonesian (and indeed Southeast Asian) sector is still largely dominated by small-scale producers, who are partially embedded within a subsistence economy. Changes in the nature of production and livelihoods in the fisheries sector appear similar to those in land-based agriculture but have received far less attention in the literature and demand further analysis given the distinct characteristics of the natural resource base. Using national datasets complemented by insights from a two-month period of fieldwork in South Sulawesi, this paper presents the process of capital intensification underpinning national fisheries growth and how it is transforming small-scale production systems. Despite increasing market integration, we found that smallholders have persisted across coastal production systems to an even stronger degree than land-based agriculture. We suggest some reasons why this is so. However, we also observed evidence of internal class differentiation within coastal communities. Such differentiation, combined with resource degradation and depletion, exposes the poorest in the community to enhanced livelihood vulnerability.  相似文献

Inclement weather creates a chronic hazard for Canadian travellers. Past studies indicate that road collision rates increase during precipitation, although the magnitude of theincrease varies from study to study, partly as a result of variations in weather and driving conditions,but also because of differences in methods. The goal of the current study is to improve ourunderstanding of the links between weather and travel risk in mid-sized Canadian cities by using astandardized method for analyzing data from six cities with different climates: Halifax-Dartmouth,Ottawa, Québec, Hamilton, Waterloo Region, and Regina. The study has four interrelated objectives: (1) Toconduct a sensitivity analysis to determine the extent to which risk estimates vary depending onthe criteria used to define precipitation events and `normal' conditions; (2) To compare therelative risk of collision and injury during precipitation relative to `normal' conditions; (3) Todetermine the extent to which weather-related risks vary fordifferent Canadian cities; and (4) To explore any differences in collision characteristics between events and controls, especially as theyvary from city to city. Results are based on a matched-pair analysis, using six-hour time blocks over afour-year period, 1995 to 1998. Results indicate only modest sensitivity to the criteria used to define precipitation events and `normal' conditions. On average, precipitation is associated with a 75 percentincrease in traffic collisions and a 45 percent increase in related injuries, as compared to `normal'seasonal conditions, but risk levels vary depending on the characteristics of the weather event.Both sensitivity to specific weather conditions and weather-related accident profiles vary from city tocity in ways that are not easily explained.  相似文献

Using reflectance values from the seven MODIS “land” bands with 250 or 500 m resolution, along with a corresponding cloud product, we estimate the fraction of each 500 m pixel that snow covers, along with the albedo of that snow. The daily products have data gaps and errors because of cloud cover and sensor viewing geometry. Rather than make users interpolate and filter these patchy daily maps without completely understanding the retrieval algorithm and instrument properties, we use the daily time series to improve the estimate of the measured snow properties for a particular day. We use a combination of noise filtering, snow/cloud discrimination, and interpolation and smoothing to produce our best estimate of the daily snow cover and albedo. We consider two modes: one is the “predictive” mode, whereby we estimate the snow-covered area and albedo on that day using only the data up to that day; the other is the “retrospective” mode, whereby we reconstruct the history of the snow properties for a previous period.  相似文献
