同型半胱氨酸水平与脑梗死患者MTHFR基因C677T多态性之间的相关性研究

目的 探讨研究同型半胱氨酸（homocysteine,Hcy）水平与脑梗死患者亚甲基四氢叶酸还原酶（MTHFR）基因C677T的多态性之间的相关性。方法 选择2012年2月~2015年8月期间在笔者医院住院就诊的450例脑梗死患者作为研究组,其中糖尿病患者181例,非糖尿病患者269例,另外选择285例笔者医院门诊部健康的体检人员作为对照组,分别采用焦磷酸测序的方法检测分析MTHFR基因C677T的多态性,并分析各基因型与Hcy水平之间的相关性。结果 脑梗死组与健康对照组的MTHFR基因型分布之间的差异具有统计学意义（P<0.05）,脑梗死组T基因的频率明显比对照组高,差异有统计学意义（χ²=13.67,P=0.00）。脑梗死组患者的Hcy浓度明显高于健康对照组,差异有统计学意义（t=12.71,P=0.00）。脑梗死组患者各MTHFR基因型患者的Hcy水平之间比较,差异有统计学意义（F=17.68,P=0.00）。非糖尿病脑梗死患者的Hcy水平明显高于糖尿病脑梗死患者,差异有统计学意义（t=2.97,P=0.00）,非糖尿病组TT型的Hcy浓度明显高于糖尿病组,CC型的Hcy浓度明显低于糖尿病组,差异均有统计学意义（t=5.67,2.18;P=0.00,0.03）。糖尿病性脑梗死患者和非糖尿病脑梗死患者中,MTHFR基因为TT型的Hcy水平明显高于CC型和CT型,差异均有统计学意义（P<0.05）,而CC型和CT型的Hcy水平之间比较差异无统计学意义（P>0.05）。结论 脑梗死患者比健康人的MTHFR基因C677T的T等位基因频率高,且脑梗死患者的MTHFR基因C677T为TT基因型时血清中Hcy的水平更高,由此判断TT基因型可能是脑梗死发生的危险因素。     

子宫动脉栓塞术治疗子宫肌瘤的临床效果及对卵巢功能的影响

目的 探讨子宫动脉栓塞术（UAE）治疗子宫肌瘤的临床效果其及对卵巢功能的影响。方法 收集2014年6月~2015年7月在笔者医院就诊的自愿接受子宫动脉栓塞治疗的子宫肌瘤患者50例,均在月经干净后3~7天内采用Seldinger技术进行股动脉穿刺,双侧子宫动脉经动脉造影证实后实施子宫动脉栓塞治疗。结果 50例患者均成功行子宫动脉栓塞术,有患者于术后1~3天均出现了不同程度地下腹痛、发热、少量阴道出血、排液等症状。持续4~6天后大多数可好转,所有患者均未出现严重并发症。所有患者于术后3、6、12个月分别行超声复查子宫与瘤体体积,术后随访患者血红蛋白、LH和HSH水平,多个时间点的重复测量资料首先采用重复测量方差分析,两两比较采用配对t检验,结果显示子宫与瘤体体积均缩小,随着UAE时间延长,子宫及瘤体体积均进行性缩小,差异有统计学意义（P<0.05）;UAE术后3、6、12个月血红蛋白水平均较术前有了不同程度的提高,差异有统计学意义（P<0.05）;UAE术前、术后3、6、12个月LH、FSH水平均无明显变化,UAE术前、术后差异无统计学意义（P>0.05）。结论 采用子宫动脉栓塞术治疗子宫肌瘤方法简单、创伤小、恢复快,并且术后对卵巢功能影响较小,是一种治疗子宫肌瘤全新的、安全有效的方法,临床上有广泛的应用前景。     

负压对四肢瘢痕切除创面成纤维细胞的生长环境影响

目的 探究持续封闭负压引流技术（VSD）对瘢痕切除创面成纤维细胞生长环境的影响。方法 回顾性分析2014年1月~2015年11月于笔者医院行植皮手术的四肢瘢痕切除术后患者60例,根据植皮手术前是否接受VSD治疗分为负压组（n=30）和包扎组（n=30）,负压组均于一期植皮前接受VSD治疗,包扎组未接受VSD治疗。回顾性分析对比两组患者并发症发生率、首次植皮存活率、创面感染率、住院时间之间的差异。并用TUNEL染色、CD34染色法对比瘢痕标本中成纤维细胞凋亡指数及血管生成情况。结果 负压组菌培养阳性率明显低于包扎组（P<0.05）,植皮存活率明显高于包扎组（P<0.05）。两组并发症发生率、住院时间、疼痛评分、随访VSS评分间比较,负压组均明显较低（P<0.05）。负压组成纤维细胞凋亡指数明显较低,微血管计数明显较高（P<0.05）。结论 负压封闭引流可以减轻瘢痕切除创面感染率,有效减轻炎性水肿,且可以通过改变局部血液循环微环境以及机械应力作用促进成纤维细胞增殖分裂,提高创面恢复效果。     

平均血小板体积变化在急性胰腺炎中的临床意义及治疗效果评估价值

目的 探讨平均血小板体积等参数在急性胰腺炎病程中变化的临床意义及其对治疗效果评估的价值。方法 回顾性分析53例轻症急性胰腺炎（MAP）患者、25例重症急性胰腺炎（SAP）患者和40例健康者（对照组）的临床资料,检测平均血小板体积（MPV）、血小板计数（PLT）、血小板分布宽度（PDW）、白细胞计数（WBC）、C反应蛋白（CRP）及血清钙离子浓度（Ca²⁺）等指标。比较并分析上述指标在急性胰腺炎病程中及治疗前后的变化。结果 与对照组比较,急性胰腺炎患者入院时MPV显著升高,差异具有统计学意义（P<0.05）;与MAP组比较,SAP组入院时MPV明显升高,差异具有统计学意义（P<0.05）;SAP组中,MPV在入院后第3天较入院时进一步升高,至入院第5天、第7天逐渐下降;SAP组中,MPV在病程中各时间点与MAP组比较均显著升高,差异有统计学意义（P<0.05）;与治疗前比较,急性胰腺炎患者治疗后MPV显著降低,差异有统计学意义（P<0.05）。结论 急性胰腺炎患者平均血小板体积与病情严重程度及治疗效果相关。     

汉族女性妊娠期高血压疾病的流行病学及发病机制研究

目的 探讨汉族女性妊娠期高血压疾病的流行病学及发病机制研究。方法 选取笔者医院10年内妊娠期高血压疾病患者689例,作为病例组,选取689例正常妊娠的女性作为对照组,采用多中心、大规模、随机队列研究方法分析汉族女性妊娠期高血压疾病患者（病例组）和对照组的资料,并通过电话随访对家属进行固定问卷式问题,对对象的年龄、孕前体重指数、孕次、产次、流产次数、孕中期平均动脉压、子痫前期史、糖尿病或肾脏疾病史、多胎妊娠等相关因素进行分析,并进行Logistic回归分析;对两组研究对象的ACE基因和AT1R基因水平、Th1/Th2比值、胎盘生长因子（PLGF）、活性氧（ROS）水平和脂质过氧化物（LPO）,分析汉族女性妊娠期高血压疾病的发病与免疫学、遗传学、氧化应激的关系。结果 年龄>35岁的初产女性、过高或过低的孕前体重指数、孕中期平均动脉压高、流产次数高、具有子痫前期史、糖尿病或肾脏疾病史、多胎妊娠的女性是导致汉族女性妊娠期高血压疾病的主要单因素（P<0.05）;多因素分析年龄>35岁的初产女性、过高或过低的孕前体重指数、孕中期平均动脉压高、具有子痫前期史、糖尿病或肾脏疾病史、多胎妊娠是导致汉族女性妊娠期高血压疾病的危险因素（P<0.05）;病例组的ACE基因和AT1R基因、Th1/Th2比值、PLGF、ROS和LPO与对照组差异有统计学意义（P<0.05）。结论 汉族女性妊娠期高血压疾病的发病与免疫学、遗传学、胎盘或滋养细胞缺血、氧化应激中的相关指标有关,而且>35岁的初产女性、过高或过低的孕前体重指数、孕中期平均动脉压高、具有子痫前期史、糖尿病或肾脏疾病史、多胎妊娠是导致汉族女性妊娠期高血压疾病的危险因素。     

2型糖尿病、非酒精性脂肪肝与血清胆碱酯酶的关系探讨

目的 探讨2型糖尿病（T2DM）、非酒精性脂肪肝（NAFLD）与血清胆碱酯酶（S-CHE）的关系。方法 随机选取221例T2DM患者,将患者分为T2DM合并NAFLD、未合并NAFLD两组。并随机选取同一时期体检的工人及退休人员206例,分为单纯NAFLD和正常对照组。所有被检者进行餐前和餐后2h血糖、糖化血红蛋白（HbA1c）、肝肾功能、血脂、S-CHE的检测以及肝胆胰脾B超检查;对T2DM患者进行空腹C肽和餐后C肽的测定,并计算胰岛素抵抗指数（HOMA2-IR）、胰岛素敏感度指数（HOMA2-%S）、胰岛β细胞功能（HOMA2-%B）。结果 DM合并NAFLD和单纯NAFLD组的S-CHE水平明显高于T2DM未合并NAFLD组和正常对照组（P均<0.01）;随着T2DM病程的延长,S-CHE、ALT、GGT、TG、TC、空腹C肽、餐后C肽的水平逐渐下降,年龄逐渐增高（P<0.05或P<0.01）;将T2DM患者分为A（S-CHE<8900IU/L）、B（S-CHE≥8900IU/L）两组,B组患者的BMI、脂肪肝百分比、ALT、AST、GGT、TG、TC、LDL-C、HOMA2-IR高于A组,HOMA2-%S低于A组（P<0.05或P<0.01）;Pearson偏相关分析显示,校正年龄、性别、病程、HbA1c、FBG、收缩压、舒张压后,S-CHE与HOMA2-IR、BMI、TG、TC、LDL-C、ALT、GGT水平呈正相关,与HOMA2-%S呈负相关（P<0.05或P<0.01）;多元线性回归分析显示,S-CHE与TC、HOMA2-IR、ALT水平呈正相关（R²=0.281,P均<0.01）。结论 S-CHE的水平在NAFLD患者明显增高,而未合并NAFLD的T2DM患者S-CHE的水平并无增高,S-CHE水平升高主要与脂肪肝、胰岛素抵抗及血脂紊乱相关,常规检测S-CHE可以反映T2DM患者胰岛素抵抗的情况以及早期预测NAFLD的发生。     

组蛋白去乙酰化酶2的表达在烟草烟雾暴露后哮喘大鼠气道Th1/Th2平衡中的作用研究

目的 通过研究烟草烟雾暴露对哮喘大鼠气道组蛋白去乙酰化酶（histone deacetylase,HDAC）2以及血浆、支气管肺泡灌洗液（BALF）中干扰素（interferon,IFN）-γ、白细胞介素（interleukin,IL）-4表达的影响,探讨HDAC2在辅助性T淋巴细胞（T helper cell）Th1/Th2平衡中的作用。方法 50只雄性Wistar大鼠随机分为对照组、哮喘组、烟雾暴露+哮喘组、布地奈德+哮喘组、布地奈德+烟雾暴露+哮喘组,建立哮喘大鼠模型、哮喘大鼠烟雾暴露模型以及布地奈德干预模型。免疫组织化学法检测各组大鼠气道HDAC2蛋白的表达,酶联免疫吸附试验法（ELISA）检测血浆和BALF中Th1型特征性细胞因子IFN-γ以及Th2型特征性细胞因子IL-4水平。结果 与对照组相比,烟雾暴露+哮喘组、哮喘组大鼠气道HDAC2蛋白表达降低,与哮喘组相比,烟雾暴露+哮喘组大鼠气道HDAC2蛋白表达降低,差异均有统计学意义（P均<0.05）。与布地奈德+哮喘组相比,布地奈德+烟雾暴露+哮喘组大鼠气道HDAC2蛋白表达降低,差异有统计学意义（P<0.05）;与对照组相比,哮喘组大鼠和烟雾暴露+哮喘组大鼠血浆、BALF中IFN-γ表达减少,IL-4表达增加,差异均有统计学意义（P均<0.01）;与哮喘组相比,烟雾暴露+哮喘组大鼠血浆、BALF中IFN-γ表达减少,IL-4表达增加,差异均有统计学意义（P均<0.01）;与布地奈德+哮喘组相比,布地奈德+烟雾暴露+哮喘组大鼠血浆、BALF中IFN-γ表达减少,IL-4表达增加,差异均有统计学意义（P均<0.01）;肺组织HDAC2蛋白的表达与血浆中IFN-γ表达呈正相关（r=0.533,P<0.01）,与IL-4表达呈负相关（r=-0.642,P<0.01）。结论 烟草烟雾暴露可以下调哮喘大鼠模型肺组织HDAC2蛋白的表达,从而影响气道炎症,IFN-γ、IL-4的表达以及布地奈德的治疗效果,这可能是烟草烟雾恶化哮喘气道Th 1/Th2平衡的一个免疫学机制。     

高脂饮食大鼠肾脏HMGCS2表达及调控研究

目的 探讨高脂饮食大鼠肾脏3-羟基-3甲基戊二酰辅酶A合成酶2（HMGCS2）表达及可能调控机制。方法 断乳Wistar 雄性大鼠27只分为3组：正常对照组（CON）、高脂组（HFG）、绿茶多酚（green tea polyphenols,GTPs）组（G）。CON组大鼠喂养普通饲料,HFG组及GTPs组给予高脂饲料,GTPs组同时自由摄食绿茶多酚浓度为1.6g/L水溶液。26周后测定空腹血糖（FPG）、血清胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白（LDL）、高密度脂蛋白（HDL）。免疫印迹法测定肾脏HMGCS2和Sirt3蛋白表达水平。结果 与CON组比较,HFG组大鼠体重和脂肪系数升高,血清FPG、TC、TG、LDL-C/HDLC均升高,差异有统计学意义（P<0.05）;与HFG组比较,GTPs降低大鼠体重和脂肪系数,差异有统计学意义（P<0.05）,GTPs组血清FPG、TC、TG、LDL-C/HDLC均不同程度降低,差异有统计学意义（P<0.05）。与CON组比较,肾脏组织HMGCS2的表达降低,差异有统计学意义（P<0.05）;与HFG组比较,GTPs组增加肾脏HMGCS2的表达,差异有统计学意义（P<0.05）。高脂饮食、正常饮食和GTPs干预后,大鼠肾脏组织Sirt3蛋白表达差异无统计学意义。结论 高脂饮食可降低肾脏组织HMGCS2表达,绿茶多酚可逆转此效应;高脂饮食及绿茶多酚对肾脏组织HMGCS2表达的调控不经Sirt3途径。     

川芎嗪对铁过载原代培养脐静脉内皮细胞超氧化物歧化酶、活性氧表达的影响

目的 探讨铁过载对原代培养脐静脉内皮细胞（HUVECS）超氧化物歧化酶（SOD）、活性氧表达的影响及川芎嗪的干预效应。方法 使用酶联免疫法（ELISA）检测体外培养铁过载脐静脉内皮细胞表达的超氧化物歧化酶和活性氧的含量。结果 与空白组比较,模型组超氧化物歧化酶的含量显著降低（P<0.01）,给药组超氧化物歧化酶与模型组比较,差异有统计学意义（P<0.05）;模型组活性氧含量显著高于空白组（P<0.01）,与模型组相比,给药组活性氧含量有明显降低（P<0.05）。结论 川芎嗪能抑制脐静脉内皮细胞铁过载导致的氧化应激,有通过抗氧化应激发挥抗动脉粥样硬化的作用。     

倾向性评分匹配法评估手术联合术中放疗在肝癌治疗中的安全性及有效性

目的 倾向性评分匹配法（Propensity Score Matching,PSM）评估手术联合术中放疗治疗原发性肝癌的安全性及疗效。方法 收集2009~2012年于笔者医院肝胆外科行手术联合术中放疗（intraoperative electron radiotherapy,IOERT）的18例原发性肝癌（primary liver cancer,PLC）患者的临床资料,其中16例经术后病理证实为肝细胞癌（hepatocellular carcinoma,HCC）,2例为肝内胆管细胞癌（intrahepatic cholangiocarcinomas,ICC）;采用倾向性评分匹配方法,按照1∶2比例从同期行单纯手术的原发性肝癌患者中筛选出36例做为对照组（32例HCC,4例ICC）,比较两组患者术中及术后并发症发生情况及HCC患者术后早期（2年内）复发率。结果 所有患者均获随访,随访率100%。两组患者术中出血量、术后3个月内肝功能、凝血功能、血常规及术后住院天数的比较,差异均无统计学意义（P>0.05）,仅在手术时长方面,联合IOERT组长于单纯手术组（P=0.000）。此外,联合IOERT组术后出现1例急性肾功能衰竭和1例胸腔积液,单纯手术组术后出现1例肝创面出血。在疗效方面,以两组48例HCC患者为研究对象,联合IOERT组的HCC患者为16例,其中6例在2年内复发,早期复发率为37.5%;单纯手术组的HCC患者为32例,20例在2年内复发,早期复发率为62.5%,两组早期复发率的比较,差异无统计学意义（P=0.101）。结论 手术联合术中放疗治疗原发性肝癌安全可行,尽管联合放疗组早期复发率低于单纯手术组,但差异并无统计学意义。
